The following table contains the various versions of the FS-ICU questionnaire. These questionnaires are available for you to use free of charge as long as you acknowledge their source and you do not modify without permission. All versions can be used for local quality improvement processes or research projects, but the FS-ICU 24R is the most updated and recommended version.
To achieve valid translations into different languages, the FS-ICU is sent to an expert for translation. It is then reverse translated back to English by another person who speaks both languages. The reverse translation is then compared to the original English version to guarantee consistency and quality.
